Recording Student Vaccinations
The SchoolBridge Permission Slip system is capable of much more than capturing caregiver consent for EOTC events. The powerful form builder allows you to capture any kind of data.
We have now enhanced the functionality to allow submitted forms to be updated over time, allowing caregivers to update vaccination information as things change.
COVID-19 Registry Form
We have created an example COVID-19 registry form that is available in the form template library. Schools may import and modify this form for their community. The form has the ability to record yes/no answers, date fields, and file uploads for attaching scans or documents.

Because of our direct connection to your SMS/KAMAR data, sending the registry form to caregivers is easy and all information submitted is authenticated and secure.
HOW IT WORKS
- Import the COVID-19 registry form and adjust as needed.
- Create a new permission slip, attach the registry form, add your students and send to caregivers.
- Caregivers receive a notification containing a link that they click and once they sign in, are taken to the registry form to fill out.
As caregivers action the registry form, your team will receive up-to-the minute reporting of the submissions with the ability to export all data. SchoolBridge also makes it easy to send reminders to caregivers that have not actioned the form or to remind them to update their information. We have also added an additional status indicator that will allow your staff to mark each submission as ‘Verified’.
Recording Staff Vaccinations
The SchoolBridge staff forms module is similar to the permission slip area and can be used to collect vaccination information from staff using a form similar to that used for students. The approval stage system will also allow you to set up a flexible verification system.
Recording Caregiver/Volunteer Vaccinations
You can use the same process to record the vaccinations of caregivers who spend time at the school or volunteer. You can do this by adding extra fields to the form you send to capture student data, or by sending a separate form to only the parents that volunteer.

Where is the data stored?
The data is stored in SchoolBridge, encrypted on our servers here in Mt Maunganui with no 3rd party access.
Who has access to the collected data?
You have complete control over who has access to the data you collect and you can provide access to only those staff that need it.
How do we control who receives the form?
The system allows you to send the form to any combination of year level, group, or subject, with the ability to add or remove individual students. You can also choose to send the form to caregivers from residence A, B, or both, and there is also an option to allow students to fill in a form.
